Abstract

Problematic cannabis use among university students represents a multidimensional public health challenge, closely linked to early substance use initiation, polysubstance patterns, and specific sociodemographic vulnerabilities. This study aims to estimate the prevalence of problematic cannabis use in a large sample of Galician university students, and to explore differential factors—particularly sex and sexual orientation—as well as co-occurring substance use patterns. A cross-sectional survey was conducted with 1,790 participants from three public universities, using validated screening instruments (CAST, AUDIT, CRAFFT). Findings revealed a 4.7% rate of problematic cannabis use, significantly higher among male students. This problematic use was associated with early onset of alcohol, tobacco, and cannabis consumption, and with frequent co-use of other substances such as stimulants, sedatives, and nicotine-based products. Although differences by sexual orientation did not reach statistical significance, an elevated risk trend was observed among LGTBI students. The findings highlight the urgent need for early, intersectional, and culturally sensitive prevention strategies aimed at disrupting substance use trajectories and promoting supportive university environments.
